Harness Clip Disengaged

Description:
It was brought to the attention of the Wind Safety Specialist that a waist strap clip on the Aspiring fall arrest harness was disengaged while climbing over the main shaft.
On the same day, another Meridian employee discovered that his leg strap was disengaged at some point and was dangling freely.
In both reported events, neither could be 100% sure if the clips were closed correctly in the beginning.
There was also another reported case at Te Apiti on 18 March 2014 of a leg strap dangling free. This event was reviewed and investigated. The outcome, in short, was the clip was not engaged correctly in the first place.
