Failed Load Ring - Industry Alert

Description:
A test lift was being performed on a bin liner with two client supplied load rings (also known as pinkies). As the bin liner has been lifted, the nut from one of the pinkies has stripped off the bolt resulting in the pinkie being removed from the anchor point while under load. The other pinkie has maintained its hold on the load allowing the crane operator to lower the load back to ground safely. The load was not fully lifted during the test lift and maintained contact with the ground at all times (approximately 70% of the load was taken by the crane). No personnel were in the immediate vicinity at time of incident.
