Turbine vibration monitoring fault

Description:
Following station start up the plant operator noted that there was no indication from one of the turbine vibration transmitters.
Investigation of this fault revealed a wiring defect at the turbine vibration element that prevented the signal from being transmitted to the station control system.
No system alarms alerted the station operator to the abnormal operating condition.
