A test lift was being performed on a bin liner with two client supplied load rings (also known as pinkies). As the bin liner has been lifted, the nut from one of the pinkies has stripped off the bolt resulting in the pinkie being removed from the anchor point while under load.

While an employee was using a chain wheel and chain to close a valve, the chain wheel retainer and chain came loose from the wheel and fell 5 metres resting approximately 1 metre from the employee, no-one was injured during the event.
